Placental abruption (placental abruption) refers to a pathological change in which the placenta in the normal position of pregnant women after 20 weeks of pregnancy and perinatal pregnant women is partially or completely peeled from the uterine wall before the fetus is born,It is a serious complication in the third trimester of pregnancy, which can lead to serious maternal and infant complications and adverse pregnancy outcomes, such as postpartum hemorrhage, DIC, acute renal failure, intrauterine fetal death, etc,if not handled properly, it often endangers the lives of mothers and babies. The clinical manifestation of placental abruption is mainly vaginal bleeding, with rapid onset and rapid development. Early detection and treatment are the key to ensuring the health of mothers and babies. At present, Doppler ultrasound has a high diagnostic accuracy rate for severe placental abruption, but the diagnostic rate for mild and moderate placental abruption is not high. Therefore, it is extremely important to explore new biochemical or immunological markers for the diagnosis of placental abruption。

 

The study collected 107 cases of placental premature abruption women who gave birth in the obstetrics department of Shenzhen Baoan District People's Hospital from March 2014 to August 2017 as the study group, aged 23-42 years, gestational age of 24+5-40+1 weeks. By detecting the changes in the expression of CA153 and PlGF in the serum of pregnant women with placental abruption, and comparing them with the corresponding indicators of normal pregnant women, preliminary exploration of the value of CA153 and PlGF in the early diagnosis of placental abruption is reported as follows.

 

 

 

Prenatal Serum PlGF Concentration in Serum of Pregnant Women with Placental Abruption

Significantly lower than normal pregnant women

 

The concentration of CA153 in prenatal serum was significantly higher than that of the control group, and the difference was statistically significant, while the concentration of PlGF was significantly lower than that of the control group, and the difference was statistically significant.   Clinical significance of PlGF and CA153 in serum of patients with placental abruption

 

Research results show that the incidence of placental abruption abroad is about 1% to 2%, and that in China is about 0.46% to 2.1%,Among them, about 1% of placental abruption maternal deaths, about 4.4% to 6.7% of perinatal mortality, and about 20% of newborns have disabilities.Therefore, early detection and diagnosis of placental abruption and timely effective treatment measures are essential to ensure the health of mothers and babies. At present, there is no ideal method or detection index to warn the occurrence of placental abruption at home and abroad. The diagnosis mainly depends on clinical manifestations and the clinical experience of doctors. Studies by Glantz et al. have shown that Doppler ultrasound has a certain auxiliary value in the diagnosis of severe placental abruption, but there is still a high rate of missed diagnosis and misdiagnosis in the diagnosis of mild and moderate placental abruption, which is mainly related to retroplacental hematoma or villi. The submembrane hematoma has not been enlarged to the extent that it can be visualized and the ultrasound imaging manifestations of placental abruption are very diverse. Diagnosis requires ultrasound doctors with rich clinical experience and technical level. Therefore, the study of new diagnostic indicators has very important clinical value in predicting the occurrence of placental abruption.

 

PlGF level can reflect the oxygen supply pressure of placental syncytiotrophoblast cells. The decrease of PlGF level can lead to placental dysplasia and placental insufficiency, and it is difficult to provide sufficient nutrients and oxygen to the fetus.It can cause fetal growth restriction and premature delivery. In severe cases, it can cause serious complications, such as stillbirth, eclampsia, HELLP syndrome, and placental abruption.The results of this study showed that the prenatal serum PLGF concentration of placental abruption women was significantly lower than that of normal women. The difference was statistically significant. This may be related to factors such as poor or incomplete placental development and insufficient oxygen supply to the fetus due to the decrease of PlGF concentration.

 

 

CA153 is a specific marker for the diagnosis of breast cancer. It has been recognized by clinical consistency in the diagnostic value of breast cancer. Its concentration increases with the development of breast cancer. It is the best indicator for clinical diagnosis and monitoring of breast cancer efficacy. And many research results show that CA153 is not only increased in breast cancer, but also in other malignant tumors, its specificity is not high, and there are many influencing factors. Therefore, it is mainly used clinically for preliminary screening of tumors. However, the results of this study showed that the prenatal serum CA153 concentration of placental abruption women was significantly higher than that of normal women, and the difference was statistically significant.Therefore, strengthening the irregular detection of CA153 and PlGF levels for pregnant women in the middle and late stages has important clinical significance in preventing the occurrence of placental abruption.
